INFORMATION CHC COLLECTS AND HOW WE USE IT

  • Your Contact Information is used to provide information to you about your CHC enrollment, programs, community resources, and educational programs/materials, public health updates, and other healthcare related information that we think may to useful to you. Contact Information includes your name, address, phone number(s) and email.  

  • Protected Health Information or “PHI”. Certain sections of HIPAA, in particular 45 C.F.R. Part 160 and Subparts A and E of Part 164 (the “Privacy Rule”), protect all “individually identifiable health information.” Your individually identifiable health information is information that (1) relates to your physical or mental health or condition, the provision of your health care, or the payment for your health care, and (2) either identifies or could be used to identify you.

    • When your “individually identifiable health information” is maintained by a health care provider or entity subject to HIPAA, such as CHC, the information is considered “protected health information.” This is true regardless of whether the information is transmitted or maintained in electronic, paper, or oral form.

    • Many common identifiers you provide to CHC, such as your name, address, date of birth, and Social Security number, are considered protected health information. In addition, your protected health information may include information about your health history, health status, symptoms, examinations, test results, diagnoses, treatments, procedures, prescriptions, related billing activity and similar types of health‐related information.

    • Throughout this Notice, “protected health information” may be abbreviated as “PHI.”

  • Payment History is used for accounting and invoicing; tracking of donations, sponsorship, registration, and other payments; and thanking you for your contributions and payments. We do not store your credit card information, and credit card payments are routed through payment card industry compliant third-party payment services.

  • Online Message Boards and similar online tools maintained by CHC allow you to disclose information about yourself publicly. Because of the public nature of these tools, other online users can access the information you provide. Be careful and discerning when posting any information on the internet, social media or other online platforms.

  •  

In addition, we may also collect other technical information such as the type of browser you are using (e.g., Google Chrome, Internet Explorer), the type of operating system you are using (e.g., Windows or Mac OS), CPU type (e.g., Pentium Intel Core), your manner of connecting to the internet (e.g., connection speed through narrowband or broadband access); internet protocol address; other information about your geographic location; and/or the domain name of your internet service provider.

COOKIES AND SIMILAR TECHNOLOGIES USED ON THIS WEBSITE

A “cookie” is a small piece of data that is sent to your browser from CHC’s web server and is stored on your computer’s hard drive. Cookies are used to collect non-identifying information about the user, such as web surfing behavior or user preferences for a specific website. CHC may use two different types of cookies — session-based and persistent cookies. Session-based cookies expire at the end of a browser session and terminate once you close your browser. Persistent cookies remain on your computer until you remove them. Persistent cookies are used to provide website analytics. You can manually delete all cookies, including persistent ones, within your browser privacy settings menu.

This website may be enabled with one or more add-on Google services, such as Google Analytics or another Google service. As such, this website will collect information consistent with the terms of Google’s Privacy Policy which could include but is not limited to the collection of device-specific information, log information, location information and cookie-related information as described in the Google Privacy Policy. Because this website is enabled with one or more add-on Google services, Google will have access to and may use Google Services Collected Information that is collected by this website and may use such information to improve Google’s services. CHC will use such information, to the extent available such as by way of Google Analytics, to better understand use of the website and topics of interest to CHC members and site visitors and may adjust our marketing practices to better target information based on such information and usage.

 

WE DO NOT KNOWINGLY COLLECT INFORMATION FROM CHILDREN

Our website and programs are intended for older adults, as defined by the Board’s criteria.  We do not knowingly collect or solicit personal information from anyone under the age of 13 without the consent by the holder of parental responsibility. If you are under 13, you are prohibited from providing information to CHC through this website without verifiable parental consent. In the event we learn that we collected personal information from anyone under the age of 13, we will delete that information and only recollect the information after receiving parental consent. If you believe that we might have collected personal information from anyone under the age of 13, please contact us using the contact information below.

PERSONAL INFORMATION WE SHARE

We do not disclose your personal information except as described in this Privacy Policy or as we disclose to you at the time the data is collected. CHC will share your personal information in certain situations:

  • Agencies, Providers and Authorized Individuals Involved in Your Care or Payment for Your Care – CHC collaborate and partner with healthcare entities to provide access, resources and care referrals to enrolled clients.  During essential communication, CHC may disclose your PHI to community providers, hospitals, agencies and authorized individuals involved in your care or the payment for your care if you verbally agree to do so, or if you are given an opportunity to object to such a disclosure and you do not raise an objection. For example, CHC may assume you agree to our disclosure of your PHI to your spouse when you bring your spouse with you to enrollment interviews and during appointments when treatment or while treatment is discussed.

  • Health Oversight – CHC may disclose your PHI to a health oversight agency for activities authorized by law. These activities include audits; civil, administrative, or criminal investigations or proceedings; inspections; licensure or disciplinary actions; or other activities necessary for oversight of the health care system, government programs, and compliance with civil rights laws.

  • Payment Processing. CHC uses third parties to provide credit card, bank, payment and information processing services. If you choose to make a payment online, by phone, through the mail, or by other means, we will share your personal information with our processing service providers. We authorize these service providers to use your personal information only as necessary to perform services on our behalf or to comply with legal requirements. We require these service providers by contract to safeguard the privacy and security of personal information they process on our behalf.

  • Business Associates. CHC may use agents, consultants, community partners and contractors to help with our mission, program and operations. These contractors are required by contract to protect your information in a manner that is consistent with this Privacy Policy and by not using the information for any purpose other than to carry out the services they are performing for CHC.

  • U.S. Member, Participant, and Sponsor/Donor List Sharing. For U.S. participants, sponsors and donors, we may occasionally share name, mailing address, email with other reputable organizations only for specific defined purposes that are intended to support our programs. You can opt out of this sharing at any time by sending an email to: info@chopeconcepts.org. Provide your complete name, email address, and/or mailing address and state your request to “Opt out”.

  • Appointment Reminders – CHC may use or disclose your PHI to remind you that you have an appointment at a CHC facility or with a CHC provider.

  • Medical Emergency. CHC will disclose your personal information to medical professionals in an emergency during outreach, a CHC-sponsored event or when CHC otherwise has reasonable presumed responsibility to do so.

  • Necessity. CHC will disclose your personal information as necessary to protect your vital interest, protect the vital interest of another person, protect public safety, respond to government requests, and report information as required by law.

YOUR RIGHTS AND CHOICES

You have rights regarding the personal information we maintain about you. We also offer you choices about what personal information we collect from you, how we use that information, and how we communicate with you.

You have the right to:

  • Limit the sharing of your information with third parties for direct marketing purposes;

  • Obtain confirmation that we hold personal information about you;

  • Request access to and receive information about the personal information we maintain about you;

  • Receive copies of the personal information we maintain about you;

  • Update and correct inaccuracies in your personal information;

  • Have the processing of your personal information restricted or object to the processing of your personal information and PHI; and

  • Have the information blocked or deleted, as appropriate.

To exercise these rights, please contact us as set forth below. You may withdraw any consent you previously provided to us. We will apply your choices going forward.

 

INTERNATIONAL DATA TRANSFERS

CHC is a U.S. based organization we do not have any members or strategic alliance partners in other countries, including Canada. We will not knowingly transfer your personal information to recipients or businesses outside of the United States.

 

INFORMATION SECURITY

We are committed to ensuring the privacy of your personal information and have established safeguards to help protect such information. For example, to protect your privacy, CHC uses passwords to help verify authorized user identity before granting access anywhere your personal information is stored. All online monetary transactions are done on a secure server managed by third party financial systems. This means that information entered on the transaction page is encrypted before transmission. Though we strive to ensure the security of your information on our systems, no security measures are perfect or impenetrable and data transmission over the internet cannot be guaranteed to be 100% secure.

 

WEBSITE LINKS

To provide certain services, we contract with other companies and individuals. These additional services and goods may include links to other websites from our website. CHC’s privacy practices may not be applicable at any third-party sites. When you leave the CHC website, we have no control regarding what information may be collected about you, nor do we control what third parties might do with the information. It is important that you review any privacy policies on third-party sites carefully before you use any services or programs offered.

 

CHANGES TO PRIVACY POLICY

From time to time, CHC may revise its Privacy Policy to address changing technology, security measures and website functionality. We will send all email subscribers an alert and/or post a prominent notice on relevant websites to notify you of any significant changes to our Privacy Policy and indicate when it was updated.
